Face

The full face is the most common area. Our doctor injects about 100 microdroplets from one 1 ml syringe across the cheeks, the lower face and the forehead, staying 1 to 2 cm between points. The first change most patients report is that skin stops feeling tight by mid-afternoon and make-up sits more evenly. In a published study of Restylane Vital on the face, 30 women received 3 sessions 4 weeks apart, 2 ml per session; doctors rated skin roughness, elasticity and brightness as the biggest improvements, ahead of moisture and fine wrinkles. It is not a treatment for deep nasolabial folds, marionette lines or hollow cheeks; those need dermal fillers.

Neck

The neck shows age early because its skin is thin and has few oil glands. Horizontal neck lines and a crepey texture respond to Vital Light, the thinner gel made for delicate skin. We inject in horizontal rows along the lines rather than a square grid. Neck skin bruises more easily than the face, so plan the session at least 2 weeks before an event. Deep, fixed bands are muscle or laxity, not dehydration, and we would discuss skin tightening instead.

Décolletage

Sun damage between the collarbones and the top of the breasts produces vertical sleep lines, crepey texture and blotchy colour. Skinboosters improve the texture and the fine lines; they do not remove pigmentation. Galderma's Vital Light study on the face, hands and décolletage found that a blinded assessor rated more than 80% of subjects as having better skin quality on the treated side throughout 9 months of follow-up. The area is large, so one syringe covers the central décolletage and a second is sometimes needed for full coverage; we tell you the syringe count before you consent.

Hands

Thin, crepey hand skin with visible tendons is one of the clearest uses for Restylane Skinboosters Vital. The product is placed as microdroplets in the dermis on the back of each hand, not as a bolus under the skin, so the hands look hydrated rather than padded. Bruising is more visible here because the skin is thin; avoid heavy lifting for a day. Both hands are treated in one session with one syringe when the skin is only mildly crepey; markedly loose skin may need two.

What a Skinbooster does not do

It does not lift, does not fill a hollow and does not change pigmentation. The 20 mg/ml gel is spread thinly by design. Patients who want a visible lift are better served by Sculptra or Utims HIFU; patients with sun spots or melasma by a laser or peel. Because it adds hydration rather than structure, Skinboosters are also the treatment we suggest first for someone who wants a change that friends notice without a change they can name.

When results appear

The gel is in place immediately, but the water-binding effect builds. Expect a first difference at 1 to 2 weeks, a clear difference 2 to 4 weeks after the second session and the full result about a month after the third. The manufacturer's data show elasticity gains for up to 6 months after the initial course; Galderma's later studies report skin quality improvements up to 15 months with maintenance. The product

Restylane Skinboosters is made by Q-Med AB in Uppsala, Sweden, part of Galderma. Vital contains 20 mg/ml of stabilised, non-animal hyaluronic acid crosslinked with BDDE, the NASHA chemistry used in Restylane fillers, tuned for a softer gel that spreads in the dermis instead of holding volume. Vital Light is the thinner version for delicate skin. Both come in a 1 ml glass syringe with the Smart Click dose guide, which clicks at each microdroplet so every one of the roughly 100 points receives the same amount, and both are supplied with lidocaine in the current formulations. Galderma reports more than 18 years of clinical use and over 9 million Skinbooster treatments worldwide.

Restylane Skinboosters or Profhilo

Profhilo is a high-concentration, non-crosslinked hyaluronic acid injected at 5 points per side of the face; it spreads on its own over the following hours. It is a quick session with very few injection points and suits a first-time patient or someone who bruises easily. Restylane Skinboosters is crosslinked, stays where it is placed and is injected point by point, so the doctor controls exactly where the hydration goes. That makes Restylane the better choice for the neck, décolletage, hands and for texture around the mouth. Our guide on Profhilo or skin boosters sets the two side by side.

Restylane Skinboosters or polynucleotides

Plinest and Rejuran Healer contain polynucleotides from salmon DNA rather than hyaluronic acid. They are used for repair and inflammation control, thin under-eye skin and acne-scarred skin, and they do not hydrate the way hyaluronic acid does. Plinest Eye is our most delivered booster because the under-eye area is where hyaluronic acid boosters are most limited. Patients with dry, crepey skin and no under-eye problem usually get a clearer result from Restylane; patients with dark, thin under-eye skin from Plinest Eye.

Restylane Skinboosters or amino acid and vitamin boosters

NCTF, Jalupro Classic and Novacutan BioPro combine a low-concentration, non-crosslinked hyaluronic acid with amino acids, vitamins or peptides. Their hyaluronic acid is absorbed within days, so they are booked as short courses at closer intervals and give a shorter-lived glow. Restylane's stabilised gel stays in the dermis for months, which is why its course is 3 sessions 4 weeks apart and then one session every 6 months.

Restylane Skinboosters or a filler

Restylane fillers are firmer gels placed deeper to add shape. A Skinbooster is a soft gel placed superficially in a grid. They can be combined in one plan: a filler for a hollow tear trough or cheek, a Skinbooster for the skin quality over it.

Before your session

Bring your skincare product list so we can confirm what to pause and when to restart. Do not stop prescribed treatment without your prescriber’s advice.

Stop aspirin, ibuprofen and other anti-inflammatory painkillers, fish oil and vitamin E for 3 days if your doctor agrees; they increase bruising. Prescribed anticoagulants such as warfarin are different: the manufacturer states the product must not be used in patients taking them, so tell us before you book. Avoid alcohol for 24 hours. Come without make-up and do not use acids or scrubs on the area for 2 to 3 days. If you get cold sores, tell us; injections can reactivate them and we prescribe an antiviral to take beforehand. Leave 2 weeks after a laser, peel or microneedling session before a Skinbooster, and 2 weeks after a Skinbooster before those treatments; the manufacturer warns that treating freshly injected skin can trigger inflammation at the injection points. Do not schedule a dental block on the same day: the gel already contains lidocaine.

After your session

Expect small raised dots at each injection point, redness and mild swelling. They settle within hours to 2 days on the face and can take longer on the neck and hands. Bruising happens and can take 7 to 14 days to fade. For 24 hours, no make-up on the area, no touching or massaging unless the doctor asked you to, and no hard exercise. For 48 hours, no saunas, steam rooms, hot yoga or direct sun; the manufacturer advises avoiding heat and extreme cold until the redness and swelling have gone. Restart acids after 3 to 5 days. Use SPF 30 or higher daily; sun on swollen skin raises the risk of dark marks, especially on darker skin.

Side effects and when to contact us

Redness, tenderness, itching, swelling and bruising at the injection points are the expected reactions and go away on their own within a few days. Less common are small lumps that can be felt but not seen; the manufacturer notes that these usually resolve within weeks but have lasted several months in rare cases, most often when too much gel was placed too superficially, which is why the dose guide and the deep-dermis placement matter. Delayed swelling or inflammation can start 2 to 4 weeks after a session; it is uncommon and treatable, and hyaluronic acid can be dissolved with hyaluronidase if needed. Galderma's post-market reporting puts inflammation, pain, nodules and swelling at 1 in 10 000 to 1 in 50 000 treatments and bruising at 1 in 50 000 to 1 in 100 000.

The serious risk shared by all injectables is injection into a blood vessel. It is rare, and rarer still with superficial microdroplets, but contact us immediately or go to an emergency department if you have unusual pain, skin that turns white, grey or blotchy, or any change in your vision. Who should not have a Restylane Skinbooster

• Pregnancy and breastfeeding: the product has not been tested in either, so we postpone. - Under 18. - Allergy to hyaluronic acid, to lidocaine, or to the streptococcal proteins used to make the gel; a history of anaphylaxis or multiple severe allergies. - A bleeding disorder, or anticoagulant or thrombolytic medicines. - Active acne, eczema, psoriasis, a cold sore or any infection in the area until it has healed. - A permanent, non-hyaluronic implant in the area. - Immunosuppressive treatment, uncontrolled autoimmune disease or a keloid tendency need a doctor's assessment before we agree to treat. - The lower eyelid in people with dark pigmented circles, thin skin or puffiness, per the manufacturer's precaution.
