Nerve-calming science that dials down pain signals and muscle tension

Botulinum toxin type A works by blocking the release of acetylcholine at the neuromuscular junction. This reduces muscle contraction in areas that commonly contribute to headaches—such as the forehead, temples, back of the head, and neck. It also modulates peripheral sensory nerves and decreases the release of pain-related neuropeptides like CGRP and substance P, which helps limit peripheral sensitization and lowers input into the trigeminal pain pathway. In practice, very small doses are placed with ultra‑fine needles using an evidence-based pattern (often called a PREEMPT-style protocol), typically across 31–39 sites chosen to match your symptoms. Injections are shallow intramuscular or subdermal, and the total dose is measured in units. This is a preventive treatment: onset commonly begins within 7–14 days, with peak benefit around 4–6 weeks, and effects generally last about 10–12 weeks.

Contraindications

You may not be a good candidate if any of the following apply. Please discuss your full medical history and medications with your clinician before treatment.
  • Pregnancy or breastfeeding
  • Active skin infection or inflammation at planned injection sites
  • Known allergy to botulinum toxin or to human albumin
  • Neuromuscular conditions (e.g., myasthenia gravis, Lambert–Eaton syndrome, ALS)
  • Bleeding disorders or use of anticoagulants that cannot be paused as advised by your clinician
  • Recent facial or scalp surgery, or unhealed wounds in target areas
  • Current use of aminoglycoside antibiotics or drugs that affect neuromuscular transmission
  • New or worsening neurological symptoms (such as sudden weakness or vision loss) that need medical assessment first
  • Unrealistic expectations or inability to attend regular follow-ups

Clear schedule, predictable relief every 12 weeks

Most people do best with a series of treatments on a steady 12‑week cycle. During your first visit, we review your headache history, triggers, and any previous therapies. We may ask you to keep a simple headache diary to track frequency and intensity. The initial session takes about 15–30 minutes, including mapping and injections. Early effects often appear within 1–2 weeks, and a meaningful reduction in headache days typically builds after 2–3 cycles. For chronic migraine, maintenance is usually every 12 weeks; for tension-type patterns, dosing and areas may be more focused, with timing adjusted to your response. Before your appointment: if approved by your prescriber, consider avoiding alcohol and unnecessary blood-thinning supplements (like fish oil) for 24–48 hours, and avoid aspirin/ibuprofen for 24 hours to reduce bruising risk; do not stop prescribed medications without medical advice. Arrive with clean skin (no makeup on treatment areas). After treatment: avoid rubbing the injected areas, heavy exercise, sauna/steam rooms, or lying flat for about 4 hours; gentle activities and normal routines are fine. Mild redness or small bruises can occur and usually settle within a few days. This therapy prevents future attacks; it does not stop a current migraine already in progress.
