Jawline and jowls

The jawline is the area where Radiesse has the strongest evidence and the most visible result. Radiesse(+) is approved for moderate to severe loss of jawline contour in adults. In the manufacturer's randomised trial, 123 patients were treated and 57 were left untreated as controls: 75.6% of treated patients improved by at least one grade on the jawline scale against 8.8% of controls, and 67.3% of the responders still had their improvement 48 weeks later.

We place the product deep, along the lower border of the mandible from the chin to the angle of the jaw. The gel sharpens the line between face and neck on the day; the collagen that follows firms the skin over the jowl. Early jowls respond well; heavy jowls with loose neck skin need Ultraformer HIFU or Endolift first and Radiesse for definition afterwards.

Chin

A short or receding chin makes the whole lower face look weak and the neck look fuller than it is. One syringe of Radiesse placed on the bone at the point of the chin projects it forward by a few millimetres and lengthens the jawline in profile. Because the product sits on the periosteum, the result feels firm, like bone, not soft like a hyaluronic acid filler.

Cheeks and nasolabial folds

With age the fat pads over the cheekbone shrink and slide downwards. The cheek flattens and the fold from the nose to the corner of the mouth deepens. Radiesse is approved for moderate to severe nasolabial folds, but we rarely inject the fold itself. Restoring the cheekbone with one syringe, half a syringe per side, lifts the skin above the fold and softens it indirectly, which looks more natural than filling the crease.

Temples and pre-jowl sulcus

Hollow temples make the eyebrow tail drop and the face look narrow at the top. A small amount of Radiesse placed deep on the temporal bone restores the oval. The pre-jowl sulcus is the notch in front of the jowl on each side of the chin; a quarter of a syringe on each side straightens the jawline without touching the jowl.

Neck, décolleté, arms and hands

Diluted Radiesse is a different treatment from filling. The 2018 global consensus on diluted (1:1) and hyperdiluted (1:2 or more) calcium hydroxylapatite lists the mid and lower face, neck, décolleté, upper arms, abdomen, upper legs and buttocks as areas where it improves laxity and skin quality. Radiesse diluted 1:2 with saline is approved for décolleté wrinkles, and undiluted Radiesse for volume loss on the back of the hands.

We inject the diluted product in a fan pattern just under the skin with a cannula. Nothing is visible on the day except mild swelling; from week 6 the skin becomes thicker and the crepe lines soften. Most areas need 2 to 3 sessions, 4 to 6 weeks apart, one syringe per session. What results to expect and when

You leave with the new contour already in place. Swelling can make it look slightly overdone for 2 to 3 days. Between week 4 and week 12 the gel is absorbed while collagen forms, so the result softens and then firms again. The photographs in our before-and-after section were taken immediately after treatment; the collagen effect is not yet visible in them. What is in a Radiesse syringe

Radiesse is calcium hydroxylapatite, the same mineral that makes up bone and teeth, formed into smooth microspheres and suspended in a water-based gel carrier. It is made by Merz Aesthetics. Radiesse(+) is the same product with lidocaine added for comfort; we use it for the face and undiluted work. Plain Radiesse is what we dilute for the neck, décolleté and hands. Both come from the official UAE distributor, and we show you the box and lot number before we open it.

Two facts distinguish it from every hyaluronic acid filler. First, it is not reversible: there is no enzyme that dissolves calcium hydroxylapatite, so the correction for too much product is time, not an injection. Second, it can show on X-rays and CT scans. It does not interfere with reading the image, but tell your dentist or radiologist that you have had it.

Radiesse or Sculptra

Both are collagen stimulators and we offer both. Sculptra is poly-L-lactic acid, a powder reconstituted in water. It gives no volume on the day, needs 2 to 3 sessions 4 to 6 weeks apart and shows its result over 3 to 6 months, but the collagen it builds is spread evenly through the whole treated area. Radiesse gives volume immediately, works in one or two sessions and builds collagen where the gel was placed.

Our rule: Radiesse for a specific structure that needs definition, such as the jawline, chin or cheekbone, and for diluted skin tightening in a small area; Sculptra for a face that has lost volume everywhere, and for large body areas. Our guide Sculptra or Radiesse compares them line by line.

Radiesse or a hyaluronic acid filler

A hyaluronic acid filler is a soft gel that holds water. It is the right product for lips, under the eyes, fine lines and anywhere you might want the option to dissolve. It lasts 6 to 12 months and does not stimulate collagen in a meaningful way. Radiesse is firmer, lasts longer, thickens the skin and is the better choice for bone-like structure on the jaw and chin. Many of our patients have both: Radiesse on the jawline and chin, hyaluronic acid in the lips and tear troughs, in the same visit or two weeks apart.

Radiesse or Ultraformer HIFU

Ultraformer HIFU tightens the SMAS layer and the skin with focused ultrasound and adds nothing. Radiesse adds structure and collagen but does not lift loose skin. For a soft jawline with little skin excess, Radiesse alone is enough. For jowls with loose neck skin, HIFU first and Radiesse 2 weeks later gives a result neither achieves alone; three of the four patients in our before-and-after section had exactly that combination.

Other skin-quality treatments we offer

For fine crepe texture rather than laxity, a Restylane skin booster, Profhilo or ASCE+ exosomes hydrate and repair the surface without adding structure. Morpheus8 radiofrequency microneedling tightens from within the dermis. We combine any of them with Radiesse, at least 2 weeks apart, when the skin surface and the structure beneath both need work.

Before your appointment

For 3 to 5 days before treatment avoid alcohol, aspirin, ibuprofen, fish oil and high-dose vitamin E unless a doctor prescribes them; they thin the blood and make bruising more likely. Do not stop a prescribed blood thinner on your own; tell us and we plan around it. Book dental work, including hygienist cleaning, at least 2 weeks before or after Radiesse, because bacteria released during dental treatment can settle on fresh filler.

Come with clean skin and no makeup. Tell your doctor about every previous filler, its brand and date, any cold sores in the area, allergies, autoimmune disease and whether you are pregnant or breastfeeding.

After treatment

Sleep with your head raised on the first night. For 24 to 48 hours avoid hard exercise, saunas, steam rooms and hot yoga, and keep the area out of direct sun. Do not press, massage or lie face down on the treated area for 3 days; the product is settling on the bone and pressure can move it. Makeup can go on after 12 hours. Facials, massage and other injectable or energy treatments in the same area wait 2 weeks.

Swelling peaks on day 2 and is gone by day 3 or 4. A bruise, if you get one, fades within 7 to 14 days and can be covered with makeup after the first day. Our side effects and recovery guide sets this out day by day.

Side effects and when to contact us

Expected: swelling, redness, tenderness, itching and bruising at the injection points, all temporary. Uncommon: a small firm nodule you can feel under the skin, more likely in the hands and with product placed too superficially, which usually softens over weeks. Rare: infection, an allergic reaction, or product visible as a lump.

The one serious risk of any filler is injection into a blood vessel. The skin over the blocked vessel turns white, then dusky or blue, and hurts more than the rest of the face; in the worst case the retinal artery is involved and vision changes. Contact us immediately on the clinic number if you have severe or increasing pain, a white, grey or blue patch, or any change in your vision, at any hour. Do not wait for the review appointment. Because Radiesse cannot be dissolved, our prevention is technique: cannula not needle, deep placement on the bone, slow injection in small amounts, and no Radiesse in the nose, glabella, forehead or around the eyes.

Who should not have Radiesse

• Pregnancy and breastfeeding; we postpone. - Active infection, cold sore, cystic acne or rash in the area; we wait until it has healed. - Known allergy to calcium hydroxylapatite or lidocaine, or a history of anaphylaxis. - A bleeding disorder, or anticoagulants that cannot be managed with your prescribing doctor. - Autoimmune disease or immunosuppression is assessed case by case with your treating physician. - A tendency to keloid or hypertrophic scars. - Dental treatment, a strong peel or laser in the area within the last 2 weeks. - Lips, under-eye area, nose, glabella and forehead are never treated with Radiesse.
