Volume: fuller upper or lower lip

The most common request is a thin upper lip that disappears when you smile. Gel placed in the body of the upper lip adds height and turns the lip slightly outward, so more of the pink shows. We keep the upper lip smaller than the lower: an upper lip that is fuller than the lower is what most people mean by "done lips". One millilitre shared between both lips gives a visible change that friends notice but cannot name.

Definition: the border and Cupid's bow

The vermilion border is the ridge between lip and skin. With age or genetics it flattens, lipstick bleeds into the lines above the lip and the mouth looks undefined. A thin thread of gel along the border, Belotero Lips Contour or Restylane Kysse, restores the ridge and the two peaks of the Cupid's bow without adding volume. This is the "lip contour filler" many patients search for. It can be combined with volume in the same syringe.

Symmetry and shape correction

One side fuller than the other, a lower lip that sits to one side, or an uneven border from an old injury: filler corrects these by adding to the smaller side only. The gel amounts are small, often 0.2 to 0.4 ml per side, so 1 ml usually corrects the asymmetry and still has volume left for the rest of the lip.

Corners of the mouth and smoker's lines

Downturned corners give a sad or tired look. A small deposit of gel under each corner supports it and lifts the line of the mouth. The fine vertical lines above the upper lip respond to a soft gel placed very superficially; Juvederm Volbella is made for exactly this. Deep smoker's lines need more than one treatment and often a Restylane skin booster in the same area.

Dark lips are not a filler problem

Search shows many patients in Dubai looking for lip lightening. Filler does not change lip colour. Pigmented or dark lips are treated with our ClearSkin lip rejuvenation laser, which targets the pigment. If you want both colour and shape, we do the laser first and inject filler 2 weeks later.

What lip filler does not do

Filler does not lift a long upper lip or shorten the distance between nose and lip; that needs surgery. It does not stop lips ageing: the gel fades and the treatment is repeated. A result that lasts longer than 18 months is usually filler that has spread, not filler that worked better.

Results over time

You see the shape immediately, then swelling exaggerates it for 2 to 3 days. The true result shows at 2 weeks, when we review you and photograph the lips. In the 48-week Restylane Kysse study, 88% of patients still met the fullness target at week 8 and 60% at week 48 after an average of 1.8 ml. What the three brands have in common

Every lip filler we inject is cross-linked hyaluronic acid of non-animal origin with lidocaine mixed in for comfort. All three come from their manufacturers, Allergan (Juvederm), Galderma (Restylane) and Merz (Belotero), through their official UAE distributors, and we show the paperwork on request.

Restylane Kysse

Restylane Kysse is the only one of the three designed exclusively for lips. Its XpresHAn cross-linking makes a gel that is firm enough to hold shape but flexible enough to move with the lip, so speech and smiling look natural. It is cleared to last up to 1 year. In its 270-patient randomised study against Juvederm Volbella XC, Kysse achieved the same lip fullness with less gel: an average of 1.82 ml against 2.24 ml. We reach for Kysse when a patient wants volume that still looks soft in motion.

Juvederm Volbella and Juvederm Ultra

Allergan makes two Juvederm gels for lips. Volbella XC is the softest: it suits fine lines above the lip, subtle volume and patients who swell easily. Ultra XC is firmer and gives more projection for lips that need a clear lift. Allergan states that results in the lips last up to 1 year and that side effects, mainly swelling, tenderness, firmness and lumps, resolve within 30 days for most patients.

Belotero Lips Shape and Lips Contour

Belotero's Cohesive Polydensified Matrix gives a gel with zones of different density, so it integrates into the tissue rather than sitting as a lump. Lips Shape, with 25.5 mg/ml of hyaluronic acid and 0.3% lidocaine, is injected under the mucosa to build volume. Lips Contour is the thinner sister product placed along the vermilion border for definition. We use the Contour gel for patients who want a sharper outline and no extra size.

How we compare them with the alternatives we offer

• Lip fillers change shape and volume within minutes and last 6 to 12 months. Reversible with hyaluronidase. - Restylane skin booster hydrates the lip and the skin around it and softens fine lines, but adds no shape. - ClearSkin lip laser treats dark or pigmented lips. It changes colour, not size. - Dermal fillers for the chin, cheeks or nasolabial folds use the same brands in firmer versions. Treating the chin at the same visit often balances the lower face better than a second lip syringe.

What the product does not tell you

The gel is a quarter of the result. Where it is placed, how deep, and how much decides whether lips look natural. Our doctors inject at the submucosal level the manufacturers specify, in small amounts per point, and stay inside the vermilion border unless a contour is the goal. Before your appointment

Stop aspirin, ibuprofen, fish oil, vitamin E and ginkgo 5 days before, unless a doctor prescribed them; they make bruising more likely. Do not stop a prescribed blood thinner on your own; tell us and we plan around it. Avoid alcohol for 24 hours. If you get cold sores, tell us at booking: we prescribe an antiviral tablet to start 2 days before, because needle trauma to the lip can trigger an outbreak that spreads over the filler.

Have any dental work, including cleaning, at least 2 weeks before or 2 weeks after. Dental treatment releases bacteria into the blood that can settle on fresh filler. Come without lipstick, and if you had filler elsewhere, bring the product name and date.

The first 48 hours

Swelling starts within an hour and peaks on day 2. Upper lips swell more than lower lips, and the first 24 hours can look uneven; that is water, not the final shape. Cool the lips with a wrapped ice pack for 10 minutes at a time. Sleep with your head raised on the first night. Avoid exercise, saunas, hot drinks, kissing and lipstick for 24 hours, and do not press or massage the lips unless your doctor showed you how. Bruising, if it happens, is usually a small purple spot at an injection point that fades in 5 to 7 days and hides under lipstick after day 2.

Days 3 to 14

Swelling drops sharply from day 3. Small firm areas you can feel but not see are normal while the gel binds water; they soften by week 2. Flying is fine after 48 hours. We review you at 2 weeks, compare photographs and, if the shape needs a small adjustment, do it then. Side effects

Expected: swelling, tenderness, small bruises and temporary firmness, lasting from a few days to 2 weeks. In the Restylane Kysse study the most frequent treatment-related events were an injection-site lump in 10% of patients, bruising in 8% and a nodule in 5%, most lasting a week or less. Uncommon: a cold sore outbreak, a lump that persists beyond 4 weeks, or filler that migrates above the lip line months later; both are treated with hyaluronidase. Rare: infection, an allergic reaction, or a delayed inflammatory nodule weeks to months after treatment.

Emergency signs: call us the same hour

Filler injected into or pressing on an artery blocks blood flow to the lip skin. The signs are severe pain out of proportion to the injection, skin that turns white, then blotchy purple, and a dusky or dark patch that spreads within hours. Visual disturbance after any facial filler is an emergency. Call us immediately at any hour on WhatsApp or phone; we keep hyaluronidase in the clinic and dissolve the filler on the spot, which reverses the blockage if it is treated early. Do not wait for the review appointment.

Who should not have lip fillers

• Pregnancy and breastfeeding: safety data does not exist, so we wait. - An active cold sore, acne or infection on or near the lips; we treat after it heals. - Known allergy to lidocaine, hyaluronic acid or the proteins from the bacteria used to make it. - A bleeding disorder or blood thinners that cannot be managed. - Autoimmune disease in a flare, or recent chemotherapy or immunosuppression; we ask your treating doctor first. - Permanent silicone or unknown permanent filler in the lips: hyaluronic acid over it raises the risk of nodules and infection. - Under 22 years of age.
