Lower third: jawline and jowls

This is the zone our patients choose most. Early jowls are a small fat pocket that has slid below the jawline plus skin that no longer holds it. The fibre passes along the jawline from an entry site behind the jaw angle, reduces the pocket and contracts the bands that anchor the skin, so the jaw edge reads as a line again. It does not reposition the deep SMAS layer, which is what a surgical lift does.

Double chin and under-chin fullness

A double chin can be fat, loose skin, a low hyoid position or a combination. Endolift addresses the first two: the fibre enters under the chin, reduces the pinchable pocket and tightens the skin from the chin to the neck. If the fullness is mostly fat with firm skin, Clatuu chin fat freezing is a needle-free alternative; if the skin is loose, Endolift.

Neck

The neck zone treats crepey, loose skin between the chin and the collarbone. A hanging fold of excess skin is a surgical question.

Full face and full face with neck

A full-face plan adds the mid-face and cheek border to the lower third, for general laxity rather than a single pocket. It uses finer fibres and more entry sites. We treat the cheeks with restraint: cheek volume is fat, and reducing it on a lean face hollows rather than lifts.

How 1470 nm laser tightens skin and reduces fat

Endolift is a 1470 nm diode laser (Eufoton LASEmaR 1500) delivered through an optical microfibre of 250 to 500 microns. The fibre is passed into the layer beneath the dermis and drawn back slowly in a fan pattern. At 1470 nm the energy is absorbed by water and fat, so the heat stays around the fibre: the fibrous septa that anchor the skin contract immediately, collagen remodelling follows over months, and small fat lobules are disrupted and cleared by the body.

A 2024 systematic review of 23 studies found favourable results for face and neck lifting, laxity and jowl fat, with adverse events described as mild and self-limiting, and called for larger controlled trials. A randomised study of 96 people aged 45 to 55 with lower-third laxity reviewed results at 60, 120 and 540 days. Those follow-up intervals are why we describe a result that develops over months rather than an immediate lift.

Endolift or Ultraformer MPT

Ultraformer MPT is focused ultrasound applied from outside at 4.5 mm to reach the SMAS, with 3.0 and 1.5 mm cartridges for the dermis. There is no anaesthetic and no mark afterwards; the lift is gradual and fat softens only slightly. Endolift works beneath the skin, reduces a defined fat pocket and tightens more strongly, at the cost of anaesthetic, entry sites and one to two weeks of swelling. We recommend Ultraformer for a lean face with early laxity and Endolift for loose skin over a pinchable pocket. Some patients have both, months apart.

Endolift or a thread lift

A thread lift repositions tissue mechanically with absorbable threads; it lifts a jowl upward rather than reducing it. Endolift reduces and tightens in place. A thread lift suits descent without much fat; Endolift suits fullness with loose skin.

Endolift or Morpheus8

Morpheus8 delivers radiofrequency through fine needles into the deep dermis and upper fat; it improves skin texture and tightens the dermis over a course of sessions, with mild fat reduction. Endolift reaches the fat layer directly in one session. For texture and pores, Morpheus8; for a jowl pocket, Endolift.

Combining Endolift with injectables

We schedule injectables at least two weeks either side of Endolift so that swelling from one does not hide the effect of the other, and we do not pass the fibre through recently placed filler.

Before your appointment

Bring your skincare product list so we can confirm what to pause and when to restart. Do not stop prescribed treatment without your prescriber’s advice.

Tell us about every medicine and supplement, especially blood thinners, aspirin, high-dose fish oil and vitamin E. Do not stop a prescribed medicine on your own; where bruising risk matters we coordinate with your prescriber. Tell us about pregnancy, breastfeeding, diabetes, autoimmune disease, bleeding disorders, keloid scarring, liver or kidney disease, cold sores, previous facial surgery, and every filler, thread or permanent implant in the lower face and neck, with dates.

Pause acids on the treated skin for three days, avoid sunburn and strong heat for two days and alcohol the day before, all of which increase bruising and irritation. Come without makeup and eat a light meal.

After your appointment

Keep the entry points dry for the first day, then cleanse gently. Wear the chin strap as advised after lower-face or neck treatment; it limits swelling and supports the skin while it settles. Sleep with your head raised on extra pillows for the first nights and use cool compresses in short intervals for the first two days. Avoid saunas, hot baths, hard exercise and facial massage until your doctor clears you, usually one to two weeks, and keep the area out of direct sun with SPF 30 or higher.

Swelling, bruising, tightness and patchy numbness along the jaw are expected. Swelling and bruising usually fade over one to two weeks; tenderness and numbness can last a few weeks. Do not book facials, peels or lasers on the zone for at least two weeks.

Review and a second session

We review at six to eight weeks and again at about three months, when swelling has gone and collagen has had time to build, comparing standardised photographs against your baseline. A second session on the same zone is a fresh decision based on that comparison, not a standard part of the plan.

Common and less common effects

Expected: swelling, bruising, tenderness, a tight feeling and patchy numbness along the jaw or neck for days to weeks. Less common: small burns at the entry points or along the fibre track, prolonged numbness, a lumpy or uneven contour while healing, skin darkening over a bruise, infection at an entry point, and a cold-sore flare. Nerve irritation along the jawline can cause temporary weakness of the lower lip. Published case series describe burns and nerve-related problems after subdermal laser treatment; they identify possible harms, not a rate for our clinic.

When to get prompt help

Contact us the same day for blistering, an open wound, spreading redness, fever, pain that increases rather than eases, or a crooked smile, drooping lip or new numbness beyond the treated zone. Do not wait for the scheduled review. Breathing difficulty or rapidly increasing swelling of the neck needs emergency care.

When Endolift is unsuitable

We do not treat during pregnancy or breastfeeding; through active skin infection, an acne flare, cold sores, open wounds, dermatitis or severe irritation in the zone; or through recently placed dermal filler in the planned fibre path. Bleeding disorders and anticoagulants that cannot be paused, uncontrolled diabetes, significant autoimmune disease, and liver or kidney dysfunction mean postponing or choosing another option. A tendency to keloid or hypertrophic scarring, allergy to local anaesthetics, and permanent fillers or biopolymers in the zone are reasons not to treat. Severe laxity is better served by surgical assessment; we say so rather than treat.

Licensing and an original device are firm standards, but they do not remove these risks.
